Appointments & Policies
- Medical Information Bring all X-rays, MRI’s, and other studies along with any reports to your appointment including any pertinent medical and/or operative reports. You will need to contact the appropriate hospital or medical office to obtain this information. If you do not have this information at the time of your visit, the physician may not be able to fully evaluate you.
- Completed Patient Information and Personal History Forms. Please download the forms here and complete prior to your visit. If you are unable to do so please contact our office.
- Insurance I. D. Card
- Please obtain a referral if needed

Office Policies
Insurance
We participate in Medicare, most other insurance plans, and many managed care and HMO’s. Always bring your insurance card(s) to each appointment with us. Please keep us informed of any changes to your phone number, address or insurance. Covered procedures will vary with each insurance company, and our office staff will assist you in determining eligibility. Patients are responsible for deductibles, co-payments and remaining balances. We accept most workers’ compensation insurance. We will be happy to help in facilitating your claim. If you have been referred to us from your primary care physician or belong to a managed care or HMO program, you are responsible for making sure you have proper authorization and referral forms for all your visits. Unfortunately without authorization, we will be unable to see you.
Payment
We will bill all primary and secondary insurances if you provide the insurance information. Co-payments are expected to be paid at the time of service. The financial responsibility for services is the patient’s obligation or his/her family, regardless of insurance coverage. We accept the following credit cards: American Express®, Master Card®, Visa®
Cancellations
If you are unable to keep an appointment, we ask that you kindly provide us with at least 24 hours’ notice by calling us at: (781) 769-6720 or (617) 696-9299. This courtesy on your part will make it possible to give your appointment to another patient.
